[LM_NET] STudent Survey of Library Media Center----4th gr---TARGET

Does anyone at the elementary school level have any surveys   they will 
share?  I am leaning toward 4th grade to obtain a student survey of the 
library.  That is the intended audience I am targeting.

I did check the lm net archives, and there were not any.  I did see the 
sample on the SC DOE web site for SLMS.
