Thank you to those who responded to my request. You're the best! I only got a few
responses and a lot of requests for a hit..

Someone named Michelle emailed me a great rubric report card that I may combine
with the information below.. It's an attachment, so if you want the attachment,
please email me and I will forward it on to you. I couldn't open it at my home.
only at my school which is strange since it's like trying to get into Fort Knox at
my school.

I've never done report cards for library before and neither has the school I just
started at this year, but we will grading next year. The report cards will
separate skill and behavior/effort. All the special are going to have the same 3
or 4 "Behaviors That Promote Learning" to grade on. I'm not sure exactly what
they're going to be. The behavior rating scale is:

4 - consistently

3 - most of the time

2 - some of the time

1 - not at this time

I came up with these as my library skills:

a.. Understands and uses library media vocabulary

a.. Selects and locates appropriate materials and information

a.. Uses materials and information effectively and efficiently

  a.. Understands and applies concepts

a.. Not sure how it will all translate to assessment.

The skills rating scale is:

4 - consistently meets, and often exceeds curriculum expectations

3 - meets curriculum expectations

2 - progressing toward curriculum expectations

1 - not meeting curriculum expectations at this time

Here is my HIT for the report card comments starting with my original post.
I continued to search the web for examples and came across this wonderful website
and with
their permission have passed it on to you. Check it out! It's almost exactly what I
was looking for.
