[LM_NET] TARGET: Template for Library Lesson

I'm looking for a blank lesson plan template for library / technology
instruction.  I've found some elementary lesson plan templates online but
they don't meet my needs.  If you have one that you've created and don't
mind sharing OR if you've discovered a website of blank (word or pdf
formats) library or technology lesson plan templates, I'd love to see it.

I'll post a HIT if others are interested.
