[LM_NET] Target - Library Materials Survey

Does anyone have a survey / input form they are willing to share that
you use
with your staff when asking for input on library material purchases? My
budget is bigger
than I could have expected for this year.  I am hoping for something
that will get more
than "Buy more books" responses.

I'll post a hit, if appropriate, and contributors are willing.
