Thanks to everyone who sent information on a form for parents to sign when they  
check out media center books themselves.  I did get a few responses.  I wound up 
not having anything for parents to sign.  We are going to just put them into the 
computer when they come to checkout.  I did wind up giving parents a brochure of 
library services offered (which had a little blurb about how students are expected 
to be responsible for books!) and hopefully some will come to use the media center!

Here's my original post with some of the responses I received:

Each family may have 1 FAMILY LIBRARY CARD.
Books are can be checked out for 2 weeks. 
You may check out up to 4 books at a time. 
This application represents an agreement between you and the school that you will 
be personally and financially responsible for any lost or damaged books.
Together we can help your children develop a love of reading as well as responsible 
habits in the care and use of books. Please discuss the importance of library 
responsibility with your child. Thank you for participating!


>>>It is time to start checking out books in the Media Center. We are very excited 
>about getting started this school year. Your child will be encouraged by his/her 
>teacher and myself to check out books from the Media Center. Please take time to 
>talk with your child about the importance of taking good care of the books and 
>returning them on time. Also, I would like to ask you to spend some time each 
>night reading and experiencing these wonderful stories with your child. 

        Your child will be allowed to check out books and will continue to be able 
to check out books throughout the school year. It is important that our children 
begin to develop a sense of responsibility, so when your child borrows books from 
the Media Center, he/she is responsible for them until they are returned. You will 
be asked to pay for any lost or damaged books checked out by your child. In the 
event your child loses or damages a book, he/she may or may not be allowed to check 
out more books. If you pay for a book lost by your child, and the book is later 
found in a usable condition, your money will be cheerfully refunded upon receipt of 
the book. 

Please sign and return this form as a record of your cooperation and consent in our 
efforts to provide your child with appropriate reading materials. This form must be 
signed and returned before your child can begin checking out materials from the 
Media Center.
