Hello All,

To assist in librarian/classroom teacher collaboration, I was thinking of creating 
a form/worksheet (online) that classroom teachers would need to fill out prior to 
bringing their classes down for a research assignment.  The classroom teacher would 
fill the form out, send it to me and then I would meet up with the teacher to 
discuss possible strategies and outcomes and thus come up with a lesson plan for 
the research assignment.

Do any of you do something similar and if so, could you explain how it works and 
possibly send me an example?
