[LM_NET] Target: equipment checkout for faculty over the summer

I have a couple of teachers who are taking summer courses and want to check
out laptops for the summer.  The principal wants me to create a
responsibility form (promising everything but their first born) for their
signature before they leave the building with the laptops.  Does anyone have
one already created I could adapt?
