[LM_NET] TARGET: Mobile Lab forms and checklists

I started to draft a form and a checklist for our new mobile laptop lab when 
I realized someone out there in LM_NET land might already have these forms.  
Why reinvent the wheel when someone has already done so (and, I'm sure done 
it in a much better way than I ever could!)

I'm looking for a form to report problems with equipment (we have laptops, a 
camera, printer, and projector).  Also, a checklist of things to check 
BEFORE signing it out and AFTER returning it (would this be the same 
checklist or two different ones?)

Thanks in advance.  I'm doing a short PD on Friday to the whole school 
regarding our new equipment, so it would be great to get this ASAP.  If you 
are wondering how we are able to get all this equipment, it was courtesy of 
the federal Improving Literacy through School Libraries grant (which opens 
around this time each year).  We are a tiny school and received a much 
needed $77,000, so I encourage everyone to go for it!
