[LM_NET] Catalog Card WORD template--->TARGET

I am looking for a catalog card template made onto a Word document. Does 
anyone have an already made template they could send to me?     Although I 
prefer the actual template already made, I would settle for directions from 
anyone who knows step by step.   This request is for a friend.
