[LM_NET] library access survey

I am wanting to send out a survey to parents (K-12) to find out what type of 
access the students have at home to computers and to public libraries.  I 
didn't know if anyone had ever come across or made their own survey for 
students or parents that I could adapt.

So far I have questions about if they have a computer at home, if it has 
Internet access, word processing, where they use computers if they do not 
have one at home, do they have a library card, what library, how often they 
use the public library.
