[LM_NET] TARGET: beg of year survey

Our new school webpage (Schoolwire) accomodates surveys. I would like to 
post one as an example for teachers but also that would be useful for me. If 
you have a beginning of year survey you would share, or just some questions, 
I would appreciate it. It could be for teachers or students.
