Hi, Angie:

I have a mini quarter-sheet form that kids can fill out when they have a request.  
They can request specific titles, genres, or topics.  They enjoy the official-ness 
of it, and the requester gets to be the first borrower, too.  I like the system b/c 
I really value the kids' input.  I also like being able to sit down with a stack at 
look at reviews, etc.
