[LM_NET] TARGET: Photo Permission Forms

Hi Everyone:

I need some ideas from others as to what you are using for photo permission forms.  
We would like to have one form that is used through out the district that parents 
sign giving permission to print their child's photo in any of the following:  
yearbook, newspaper, school newsletter, web, etc.

I would love to see what some of you have.  Thanks a million!
