[LM_NET] GEN: library assistant evaluation

It is time for our yearly evaluations of our assistants.  Our school library 
currently uses the same evaluation tool that is used for the regular classroom 
assistants.  I would like to use one that is more specific to the job of the 
assistant librarian.  I have a new assistant this year, and I would very much like 
to use this evaluation tool as a springboard for discussing improvements that we 
need to make. 

If anyone has an evaluation form that you use for your assistant librarian, I'd 
love to see a copy.
