[LM_NET] SEC: Collaborative lesson plan Template

I have spent hours searching lesson plan sites, librarian sites and even the 
archives of LM_NET. I am looking fora template I can use when a teacher comes to 
collaborate with me. I want to be able to plug-in objectives,materials, 
step-by-step procedures, etc. that would help us 
work together and "delegate" responsibilties between  us in a more "formalized" 
way. I think it would be more professional to sit down with a teacher have the 
discussion then later be able to email or hand him/her a  document ( now this 
sounds too formal) that sets out what we are both going to do for this class 
period(s).  Does anyone understand what I am seeking, and does anyone have a 
resouce for me?
