[LM_NET] TARGET: FORM for Video tape & showing of program across CC TV

I am looking for a form which teachers complete when they want me to tape a program 
via cable and then show either to their class OR across a closed circuit television 
system.  A form whereby the principal approves the taping and showing is what I am 
especially wanting.  A form that doesn't will work too.
