Student Assistants

My apologies to the group--I know this topic has been revisited numerous =
times, but since it is now knocking on my front door I am appealing to the =
group for help!

I have been told to make an application and list my requirements for Librar=
y Assistants.  I need to do it ASAP and so, rather than reinvent the wheel =
I am asking for anyone who has a ready made model to please email it =
directly to me. =20

I am in a middle school and this elective is open to 6-7-8 graders during =
their elective time. =20
