GEN: Evaluation Form for Subscription Databases

["debwaugh@frontiernet.net" <debwaugh@FRONTIERNET.NET>]

I'll be evaluating some subscription databases for my county with the
evaluations to be used in our funding decisions this Spring. Does
anyone have a form for this sort of evaluation that I could use?
